Prescription Item Requirements
Federal Law requires that prescription drugs and supplies to be restricted to use by or on the order of a licensed veterinarian. Hagyard Pharmacy endorses the American Veterinary Medical Association policy that drug therapy, when medically indicated should be initiated ONLY by an attending veterinarian in the context of a valid veterinarian-client-patient relationship.
Before we can process your prescription medication order, we must have a valid prescription from YOUR veterinarian on file.
1- Your veterinarian may call in the prescription to one of our pharmacists: (859) 255-0308
2- Your veterinarian may FAX the prescription to us from his/her office: (859) 388-9331
3- If your veterinarian provided you a written prescription, you may mail it to:

4- If we have a refill for your animal’s medication on file, you may place your order & we will process it as long as the prescription remains valid.
The ENTIRE order will be held pending the veterinarian’s authorization. If you would like to place an order before your veterinarian calls or faxes the prescription, you may do so. Our customer service representatives will place the order on hold until we receive the valid prescription.
As soon as the prescription is received, the order will be released and processed.
We are unable to allow the return of prescription drugs due to federal and state regulations.
